- Copy of Resolution
- Lit of Authorized Signers including name, address and social security number for each
- Copy of Articles of Incorporation
- Copy of By-laws
- Certificate of Good Standing (if available)
Limited Liability Company (LLC)
- Charter
- Fictitious Name Certificate (if applicable)
- Tax ID Number
- Copy of Resolution
- List of Authorized Signers including name, address and social security number for each
- Copy of Articles of Organization
- Copy of Operating Agreement
- Certificate of Existence (if available)
Partnership
Organization/Club/Church
- Tax ID Number
- Documented minutes from business meeting
- Certification and authorization by the Secretary
- By-laws of organization (if applicable)
